Medical Identity Theft Monitoring Services

Under the terms of the Settlement Agreement approved by the Court, you are eligible for five (5) years of Medical Shield Total monitoring services. Enrollment is now open for Medical Shield Total. You will receive five years of Medical Shield Total service from the date of your enrollment.

To enroll in Medical Shield Total, visit app.medicalshield.cyex.com/enrollment/activate/camcms and enter the activation code, which was provided to you on the settlement postcard notice you were previously mailed. Please note this code is unique to you and should not be shared with anyone else for enrollment. In addition to your activation code, you will need to provide your name, address, date of birth, telephone, a valid email, and your Social Security number to complete your enrollment. The provision of this information is necessary to provide you with the full benefits of Medical Shield Total.

The deadline to enroll is October 30, 2025. After October 30, 2025, the enrollment process will close, and your Medical Shield Total code will no longer be active. If you do not enroll by October 30, 2025, you will not be able to take advantage of Medical Shield Total, so please enroll before the deadline.
